Direct Primary Care (DPC) is a simple, transparent membership model. You pay a flat monthly fee directly to your doctor or nurse practitioner. In return, you get unlimited access to primary care — no copays, no claims, no hold music.

Dr. Jason Wesley has been board-certified in Family Medicine since 2001. He earned his medical degree from the University of Oklahoma School of Medicine and completed his internship and residency at Good Samaritan Family Practice Residency in Phoenix, AZ. After residency, Dr. Wesley served as Medical Director of the Grand Canyon Clinic on the South Rim — an experience that sparked a lasting love for the mountains and canyons of Northern Arizona, and where he met his wife Angie. They have called the Verde Valley home since 2007. Dr. Wesley spent several years in emergency medicine at Sedona Medical Center and Verde Valley Medical Center before transitioning to primary care with Northern Arizona Healthcare. In 2015, he founded and directed the Verde Valley Medical Clinic Occupational Medicine office, where he performed annual physicals for firefighters from departments across the region, including Sedona, Verde Valley, Cottonwood, Blue Ridge, and Pine-Strawberry. That work deepened his commitment to wellness and disease prevention — particularly around the cardiovascular, respiratory, and stress-related risks that come with high-demand occupations. It also reinforced what drew him to medicine in the first place: helping people live healthier, more fulfilling lives. In 2021, Dr. Wesley founded Helios Health AZ, bringing that philosophy to a broader patient community through the Direct Primary Care model — an approach that allows for unhurried, relationship-centered care focused on long-term health and prevention.

Family Nurse Practitioner
Elli Bourque is a board-certified Family Nurse Practitioner and Doctor of Nursing Practice who has cared for patients at just about every stage of life — from premature newborns in the neonatal intensive care unit to families in rural primary care communities. That range has given her a rare depth of clinical instinct and a genuine appreciation for meeting patients wherever they are in life. A Louisiana native, Elli earned her Bachelor of Science in Biology from Loyola University New Orleans before completing her BSN, MSN, and DNP at the University of Louisiana. Her graduate training sharpened both her clinical expertise and her commitment to evidence-based, patient-centered care — values she carries into every patient encounter. Elli and her husband first visited Sedona on vacation and, like so many before them, simply never wanted to leave. In 2023, they made it official, relocating with their two sons and planting roots in the red rocks they'd fallen in love with. She joined the Helios Health AZ team in 2025 and has found Direct Primary Care to be a natural fit for the kind of unhurried, relationship-driven medicine she has always believed in. Off the clock, you'll find Elli on the trails — hiking with her family and their dog, soaking in the landscape that brought them all here in the first place.

Angie Wesley is a Registered Nurse and Practice Manager of Helios Health AZ, bringing over three decades of clinical experience and a genuine passion for patient-centered care. She began her nursing career in Labor and Delivery in 1993 after graduating from Georgia Baptist College of Nursing in Atlanta, Georgia, and became a travel nurse in 1997 — a path sparked by a life-changing river trip through the Grand Canyon that drew her west for good. From 1999 to 2001, she worked at the Grand Canyon Clinic providing primary care, where she also met her husband and Helios Health AZ's medical provider, Dr. Jason Wesley. Together they settled in Sedona in 2007, where they have raised their son Leo, now 20, and put down deep roots in the Verde Valley community. Angie brings that same spirit of adventure and care to her work every day — whether she's streamlining practice operations, supporting patients through complex decisions, or making sure every visit feels personal and unhurried. When she's not at the practice, you'll likely find her on a trail in Sedona or the Grand Canyon, experimenting in the kitchen, or on a sailboat somewhere beautiful with Jason.

Alana is a Verde Valley native, born and raised in the community she is proud to serve. She has worked in healthcare for the past nine years as a Certified Medical Assistant, Certified Nursing Assistant, and Phlebotomist, gaining extensive experience caring for patients and supporting their health and wellness. Alana has a strong passion for dermatology and sports medicine and hopes to continue advancing her education with the goal of becoming a Physician Assistant in one of those specialties. She is dedicated to providing compassionate, patient-centered care and enjoys building meaningful relationships with those she serves. Outside of work, Alana enjoys hunting, kayaking, gardening, organizing, cooking, and spending quality time with her family.

Throughout her career she has developed extensive hands-on clinical skills, including assisting with skin cancer removal procedures and wound care. She is known for being friendly, outgoing, knowledgeable, and compassionate, and takes pride in helping patients feel comfortable and cared for while providing high-quality medical support. Outside of work, she is a proud wife and mother of two wonderful children. Her husband works in physical therapy, and together they enjoy spending time with their family and their dogs. She relocated to the Verde Valley from the Oregon coast 13 years ago and has embraced the beauty of the region ever since. She loves traveling, attending reggae concerts, and making the most of the beautiful outdoors. Whether at work or in her personal life, she strives to bring positivity, kindness, and enthusiasm to everything she does.

Sydney Hart was born and raised right here, and she has absolutely no desire to leave this beautiful place she calls home. She has been in healthcare for the past seven years, working in telemetry before joining our Helios Health AZ team. It never ceases to amaze her just how much there is to learn and love about this field. When she's not at work, you'll find Sydney at home with her animals — three dogs, chickens, two mini donkeys, and her horse. If they're not home, they're off in the mountains at a bowshoot or simply relaxing among the pines. We are thrilled to share that Sydney was recently accepted into a Radiologic Technology program, which she will begin this fall. We can't wait to see all the great things her future holds!

Everything you need to know about Direct Primary Care at Helios Health AZ.

DPC is a relatively new way to deliver high-quality, comprehensive healthcare from your doctor or nurse practitioner for a low, flat monthly fee.

DPC clinicians have chosen to break away from the current expensive, inefficient, complicated fee-for-service healthcare system to provide medical care at a reasonable, transparent price. We work for you — the patient — and leave out the middlemen (i.e. insurance companies and corporate employers).

Practically speaking, DPC is a membership model for healthcare. Every patient pays a flat monthly membership fee as a member of the practice. Memberships are also payable annually at a 5% discount.

First and foremost is direct access to your doctor or nurse practitioner. You can expect same-day or next business day appointments, along with a phone number and email address to contact us directly. Text messaging and telehealth visits are encouraged — and you won't be charged extra for these services.

Many issues can be handled over the phone, via telehealth, text, or email, saving you valuable time without having to come into the office. For issues that do require an in-person visit, you can expect to spend as much time as necessary to address what's at hand.

This differs significantly from a fee-for-service model, where clinicians need to see 18–20+ patients per day, leading to brief, rushed 15–20 minute appointments — rarely enough time to manage most medical problems effectively.

You won't pay a copay for any visit. You'll also be able to avoid many visits altogether by reaching us directly via text or email. We've negotiated discounted rates for prescription medications, blood tests, X-rays, and other imaging on your behalf.

Most DPC patients pair their membership with a high-deductible, low-premium insurance plan — covering major health events while leaving routine care to us. This can significantly reduce your monthly insurance premiums.

Another major savings opportunity: avoiding expensive ER and urgent care visits. It's estimated that 2/3 of all ER visits are non-emergent. In Arizona, the average ER bill is nearly $2,300. With almost 10 years of ER experience, Dr. Wesley is well-positioned to advise you on what truly requires emergency care — and what doesn't. Save one ER visit over 2½ years and your membership has paid for itself.

They share some similarities, but there are important differences. Concierge practices often bill your insurance in addition to charging a monthly fee — meaning they're still part of the insurance industry's complex reimbursement system. This is reflected in the price: the average concierge practice charges $200–$300 per month.

DPC memberships are a fraction of that cost because we don't spend resources fighting with insurance companies to get paid. We work directly for you.

You can cancel your membership at any time with a 30-day written notice. If you decide to rejoin the practice at a later date, written approval is required along with a $225 re-enrollment fee.

Medicare patients are welcome to join our practice. However, Helios Health AZ has opted out of Medicare. This means that neither the physician nor the patient can bill Medicare or seek Medicare reimbursement for services provided through our practice.

Medicare patients can still use their benefits for other healthcare needs, such as labs, imaging, and specialist visits.

At this time we do not offer immunizations. Maintaining a vaccine inventory requires significant resources, but immunizations are readily available at local health departments and pharmacies.

We care for a wide range of issues — from common acute problems like colds, flu, strep throat, and minor injuries, to chronic conditions such as diabetes, high blood pressure, and obesity.

We also offer hormone therapy using oral, topical, and pellet delivery options.
